Before a child starts to attend a kindergarten, a statement concerning the child's health shall be presented. If the child has attended the local mother-and-child clinic for regular checkups, such statement may be provided by the child's parents/guardians. Cf. The Kindergarten Act, § 23

The health declaration form  is to be submitted to the kindergarten where the child has been assigned a place. Private kindergartens may have their own forms you have to fill out.

Parents are required to make it possible for children to participate in regular health chekups. Cf. The Patients’ Rights Act § 6.1

Children have the right to necessary medical care, also in form of medical checkups in the municipality where they temporarily or permanently reside. Cf. The Health and Care Services Act § 3-2.
